I’m troubleshooting a DS that scans twice no pickup after making a selection. One of the OA2’s does not light. I replaced with known good OA2 still no light. Am I looking at a transformer issue?

Others may know better than me. But.... Has the pulse amp been rebuilt? I think there's a cap or resistor that is troublesome. Or possibly an issue in the pricing unit. I had a U100 that had a contact issue in the credit unit. Once fixed...made every selection.

Because the OA2 isn’t lighting I’m leaning towards a voltage issue. It didn’t pickup loading the tormat with a D cell

Do you have the manual with schematics? I would first look around the 0A2 that doesn't light for a bad resistor. If no manual/schematics, get some. They may seem expensive, but the time you will save will pay for them.

If these are original gas 0A2—not the Zener-in-a-tube, they must both be lit purple.
If only one is lit but there is normal voltage from the 6X4, you will have excessive write-in voltage. This causes all of a number and all of a letter to be written.
If neither are lit look for defective 6X4 or open resistor between the two filters.

As I understand this, one tube is lighting and one is not. And it seems to me that "no change" could be taken to mean either the same tube stayed unlit, or the same position stayed unlit. I think that's the reason for trying again, for clearer communication!

I swapped both AO2’s The unlit one worked in the other socket. The working tube did not light in the suspect socket. Apologies if was unclear

Sounds like you don't have the over 300VDC power that is needed. The OA2s each light up when they regulate at 150VDC, if only one OA2 lights up, then the B+ is less than 300VDC so you have to solve that. Start with the B+ filter caps and the 6X4 tube.
