In many applications, 6X4 tubes are at risk of heater-to-cathode shorts because the heater is near ground and the cathode is at B+. In the TSU-5 this isn't the case as the heater and cathode are at the same potential.
I'd closely examine the 6X4 socket for a short between pins 6 and 7. What's the resistance to chassis from the negative side of C514 with power off? You'll see it rise slowly due to the action of C515 and C512.
