It was my understanding that form 605 was always the form used for amateur radio license applications and that the only revision here is the question about felonies. Is that not correct?

In a way it kind of makes sense. 99.99+ percent of FCC licenses are for transmitters. A very small percentage are for people. Grouping the "people" license application onto a common form is likely to scale better for FCC. I'm kind of surprised to see GMRS in there as those licenses are for specific frequency use much like part 90. Maybe a throwback to when it was Class B Citizens Band.
